The feedback from the questionnaires will help shape future health and wellbeing developments for the population of Whitburn. A project is taking place in Whitburn to identify how individuals use or access local support to stay healthy. This questionnaire has a focus on physical activity and food and health which can link to reducing long term health conditions such as diabetes. To help the project team gain the views of people local to Whitburn, could you please answer these questions either as an individual or as a group. If undertaken as a group please be aware that some questions ask for individual responses, and the facilitator will be required to capture the answers. You will not be asked for your name but the information you give will help the project team identify the current picture for Whitburn. The project team is a partnership between West Lothian Council, NHS Lothian and Third Sector organisations.
